New patients: You will receive a text message three days before your appointment that will contain all the registration forms for you to complete.
Standard school/camp forms can be accessed and printed for free from our
Patient Portal at any time.
Other daycare, school, sports, and camp forms can be completed for a $10 fee with a turn around time of approximately 3 business days. If a form is needed sooner than that, we will make every effort to complete a form within 24 hours and a $25 rush fee will apply. Please complete all of the parental sections of the forms prior to presenting to our staff. If you bring forms to a well visit, we will try to fill them out during the visit if time permits. Forms brought to a sick visit, will be directed to the clinician who did the last well visit and will be returned within 3 business days.
Driver's forms require a visit and there is a $25 fee.
FLMA forms require a visit and a visit charge will apply.
